Pulsiv8 Launches “Swift SOAP”: Flat-Rate, AI Charting That Streamlines EHR Charting for Physicians

Pulsiv8 Inc. (Head Office: Chuo-ku, Tokyo; CEO: Kazuhisa Tsuji) today announces the launch of Swift SOAP, an AI system that improves the efficiency of EHR documentation in clinical settings.

By combining speech-recognition AI with generative AI, Swift SOAP automatically drafts fast, accurate chart notes and streamlines daily outpatient work. In a general internal-medicine clinic that sees an average of 65 outpatients per day, Swift SOAP is expected to reduce EHR documentation work by about 1.1 hours per day. It can also help shorten visit times, increase patient intake and revenue, and reduce the burden on both physicians and patients during peak hours.

Swift SOAP is designed to work with any cloud-based or on-premises EHR, making it easy to add to existing workflows. As many providers consider new EHR adoption in light of the Ministry of Health, Labour and Welfare’s guideline (in Japan) toward nationwide adoption by 2030, Swift SOAP can also reduce the operational load that accompanies EHR rollout.

Swift SOAP runs in three simple steps using an audio-capture device (iPhone or iPad) and a clinic PC connected to the EHR. Start recording in the Swift SOAP app during the exam; stop recording when the visit ends; in about 10 seconds, a SOAP-format summary appears on the PC for the physician to review and paste into the EHR.

While recording, the speech-recognition AI transcribes the physician-patient conversation in real time; after recording, the generative AI summarizes it into the SOAP format. (The system-captured conversation and the Swift SOAP summary output are shown in the example figure.)

Swift SOAP is available on an unlimited, flat-rate plan at ¥16,500 per physician (user) per month, tax included.

To accelerate adoption across as many clinical sites as possible and deliver value to more physicians, pricing is set lower than comparable charting-efficiency services in the market—without compromising functionality. Swift SOAP incorporates state-of-the-art speech recognition and generative AI and evolves flexibly based on feedback from the field.

In the initial interviews on chart-creation support needs, 73% of respondents rated their need at 7 or higher on a 10-point scale, and 36% rated it 9 or higher, indicating very strong demand (n=11).

Through hundreds of hours of interviews and user tests, we kept only the features that are truly useful in clinical practice. To let as many physicians as possible adopt the system with confidence, we realized a ¥16,500 (unlimited, flat-rate) monthly price point.
